Before Your First Appointment

Seven to ten days before your first appointment, would you please telephone the office (01737 226338) to confirm that you will be coming, leaving a message on our answerphone, if necessary.

If for any reason you need to cancel, please inform us immediately so that your appointment can be offered to someone on the waiting list, (failure to do so may have to incur a charge).


Questionnaires

Questionnaires are an invaluable means of collecting a lot of factual information and can save a considerable amount of time during the consultation, thus allowing us more time to focus on other important areas. 


ALL of the following questionnaires must be completed by new patients and posted to my office well ahead of your appointment.


The questionnaires are quite demanding in terms of the amount of time and effort required to complete them - allow yourself several days to do the work required.  You may need to contact other people (eg. relatives, your GP, specialists) for some of the information requested.  All information you provide is, of course, treated as strictly confidential.


The New Patient Questionnaire asks for:

  1. Basic Registration Details, including: address, phone numbers, email, GP and specialist details, etc.

  2. All your Problems - details, severity and dates of onset.

  3. Your Medications - nutritional supplements, drugs, botanicals, homeopathic medicines, etc.

  4. Details about your Life Style - diet, alcohol, tobacco, social drugs, exercise, relaxation, sleep, occupation, relationships, hobbies, stresses, childhood, etc.

  5. Your Allergies/ Sensitivities/ Intolerances (in brief).

  6. A detailed Past Medical History - as one chronological list of all health related and major personal events in your life from birth to the present. 

  7. A detailed Family Medical History - the health issues and ages of death of your immediate relatives - parents, grandparents, siblings, aunts and uncles, and your own children.


I now ask for the following sections of the questionnaire to be word-processed (as simple lists and as text documents) and emailed to me before your appointment (instead of completing these sections in writing in the questionnaires):

  1. All your Problems.

  2. All your Medications.

  3. Your Past Medical History.

  4. Your Family Medical History.


The Diet Diary is a great help to see more clearly your eating habits on three days that are as representative as possible of your usual weekday eating and drinking.


The Ecological Questionnaire concerns mainly allergies/ sensitivities/ intolerances, environmental toxic and microbiological exposures.


The Symptom Questionnaire is a check list of symptoms - it saves me going through the list verbally with you.


The document Symptom Description is not a questionnaire, but provides you with information on how to think about and describe your symptoms in detail.  If you wish to have homeopathic treatment it is essential that you understand the depth to which we like to have an understanding of your symptoms and nature.


What to Post before your First Appointment:

  1. All four Questionnaires.

  2. Copies of any Investigation results - preferably all those of the last year or two - eg. blood tests, x-rays, scans

  3. Copies of any relevant Specialist letters.

  4. A letter of referral from your family doctor, if available.


What to Email before your First Appointment:

Word-processed documents, as simples lists, emailed to me well before your appointment (instead of completing these sections in the questionnaires):

  1. All your Problems.

  2. All your Medications.

  3. Your Past Medical History.

  4. Your Family Medical History.


What to Bring to your First Appointment

  1. All your medications: drugs, medicines, dietary supplements, creams, drops, lotions, etc. that you are currently taking.

  2. A photograph of yourself (or your child, if bringing your child) to go in your medical records.  This is very helpful for bringing you to mind when I am working on your case without you present.

  3. A spare adult - if bringing a child under seven years old.


Consultation Duration

Your first appointment as a new patient is usually for 90 minutes (unless you wish to book for longer).   If your case is complex or you have many problems (past and present), a further appointment may be required to make a full enough assessment to initiate appropriate investigations and treatment.  If you feel that the latter may be applicable to yourself, you are welcome to discuss it with my staff and book a longer initial appointment in advance. 


The Consultation - What to Expect

My aim is to understand your concerns and health issues, in depth and breadth, and to identify ways of addressing them and resolving your problems. I try to achieve a true overview, and identify the majority of the problems and the factors that may be contributing to them - to see the causes as well as the effects. 


The majority of the patients I see say that this has been the first time that anyone has ever taken the time:

  1. to really listen to them

  2. to really try to see the whole picture

  3. to really try to understand why they have their problems

  4. to really try and find a solution that addresses fundamental causes rather than just suppress, control or manage symptoms


The most effective treatments are those that also address fundamental causes - and the latter may not always be obvious. 


Restoring genuine health is very much a process that usually takes time - and is usually dependent on obtaining a clearer and clearer understanding of the real issues. 


Ecological vs Homeopathic Consultation

The process of taking/receiving an ecological history is somewhat different for that of a homeopathic history.  If you are clear which approach you are more interested in, please let me know at the outset as it will influence my approach and the type of information and understanding I am seeking. 


Please be clear that it is very rarely possible to obtain sufficient information for both approaches in one consultation, particularly when you first consult me as a new patient.



An Ecological Consultation

I will always wish to cover the following information:


  1. Current/ presenting symptoms/ problems, including when each started, the circumstances and/or triggers when they started, to what extent they vary and what factors appear to influence them (make them better or worse) and whether they vary in unison with other symptoms.

  2. Previous investigations, specialist assessments and treatments.

  3. Current medications, drugs, supplements, creams, etc. and any other current therapies.

  4. Dietary history - your current and previous eating habits and the details and results of any previous exclusion diets.

  5. Allergies, sensitivities/ intolerances - drugs, vaccines, foods, inhalants (airborne), contact (skin), electrical (EMF).

  6. Environmental history - exposures to potential toxins.

  7. Microbiological history - previous infections, fevers, insect bites, etc - particularly recurring or severe ones.

  8. Psychological history and sleep pattern - any major stresses and psychological traumas throughout your life.

  9. Physical exercise/ activities.

  10. A detailed succinct chronological past medical history from conception to the present including all of the above.

  11. A detailed family history - this often provides some insight into possible inherited susceptibilities.

  12. A physical examination - the extent depends on your symptoms.


You can now see why this process may take longer than one or two hours!
