At the moment I am only accepting the occasional new patient - and this is totally dependent on appointment availability.


When you book your first appointment you will be emailed confirmation of your appointment, a new patient questionnaire in the form of a spreadsheet (Apple Numbers or Microsoft Excel), together with instructions, practice information, a consent form, a map and directions to the clinic.


Before Your First Appointment

Seven to ten days before your first appointment, would you please telephone the office (01737 226338) to confirm that you will be coming, leaving a voice message on our phone, if necessary.

If for any reason you need to cancel, please inform us immediately so that your appointment can be offered to someone on the waiting list, (failure to do so may have to incur a charge).


The New Patient Questionnaire

Questionnaires are an invaluable means of collecting a lot of factual information and save a considerable amount of time during the consultation, thus allowing us more time to focus on the most important areas. 


The Questionnaire is now in the form of a spreadsheet. It is created in Apple Numbers, which is the ideal format to use if you have an Apple Mac or iPad.  It is also available as in MS Excel but this is not as ideal as the conversion from one to the other is not perfect and modifies some of the formatting.


The questionnaire is quite demanding in terms of the amount of time and effort required to complete them - allow yourself several days to do the work required.  You may need to contact other people (eg. relatives, your GP, specialists) for some of the information requested.  All information you provide is, of course, treated as strictly confidential.


The purpose of the questionnaire is to obtain as clear an overview as possible of:

  1. 1)Your Physical and Psychological Health and Well Being, and

  2. 2)As many of the Major Factors that may be Contributing to your these.


The questionnaire asks you for details about a number of vital areas, including your:

  1. Basic Registration Details - address, phone numbers, email, next of kin, occupation, GP and specialist details, etc.

  2. Symptoms and Problems - details, severity and dates of onset, etc.

  3. Current Medications - nutritional supplements, drugs, botanicals, homeopathic medicines, etc.

  4. Past Medical and Personal History - as a list of all health related and major personal events in your life from birth to the present.

  5. Family History - the health issues of your immediate relatives - parents, grandparents, siblings, aunts and uncles, and your own children.

  6. Allergies, Intolerances and Sensitivities

  7. Energy Level

  8. Psychology - stresses, mood, self-esteem, interests and hobbies, your childhood, etc.

  9. Sleep Pattern

  10. Activity Level and Exercise

  11. Diet and previous Dietary Experimentations

  12. Current and previous Infections and Infectious Exposures

  13. Current and previous Toxic Exposures (including EMF) - including alcohol, tobacco, social and prescription drugs, environmental toxins, etc.



What to Bring to your First Appointment

  1. Your completed Spreadsheet Questionnaire - on a USB memory stick or external (USB) hard drive - unless you email it a few days beforehand.

  2. Copies of Investigation Results, particularly all from the last year or two, especially if abnormal.

  3. Copies of all relevant informative Specialist Letters.

  4. A Referral Letter from your family doctor, if available.

  5. All your Medications: drugs, medicines, dietary supplements, creams, drops, lotions, etc. that you are currently taking.

  6. A photograph of yourself (or your child, if bringing your child) to go in your medical records.  This is very helpful for bringing you to mind when I am working on your case without you present.

  7. A spare adult - if bringing a child under seven years old.



Consultation Duration

Your first appointment as a new patient is usually for 90 minutes (unless you wish to book for longer).   If your case is complex or you have many problems (past and present), a further appointment may be required to make a full enough assessment to initiate appropriate investigations and treatment.  If you feel that the latter may be applicable to yourself, you are welcome to discuss it with my staff and book a longer initial appointment in advance. 



The Consultation - What to Expect

My aim is: 1) to understand your concerns and health issues, in depth and breadth, and within the context of your life as a whole, 2) to try to identify the main factors that may be contributing to them, 3) to help you understand and address the causes (and their effects) as far as possible, in as natural ways as possible, 4) to suggest other therapeutic options that may be helpful that are not part of my skill set, and 5) to provide support you in the process.


The majority of the patients I see say that this has been the first time that anyone has ever taken the time:

  1. to really listen to them

  2. to really try to see the whole picture

  3. to really try to understand why they have their problems

  4. to really try and find a solution that addresses fundamental causes rather than just suppress, control or manage symptoms


The most effective treatments are usually those that also address fundamental causes - and the latter may not always be obvious. 


Restoring genuine health is very much a process that usually takes time - and is usually dependent on obtaining a clearer and clearer understanding of the real issues.  The best way to view this process is as a journey, as an adventure, towards progressively greater and greater understanding yourself.  We are all utterly and totally unique on all levels - mentally, emotionally, physiologically, biochemically, genetically - and the precise constellation of reasons for any particular problem is unique to us.  Almost every man-made product comes with an instruction manual - we do not!  One way of viewing the journey of our life is that it is a process of self observation, experimentation and discovery so that we are effectively writing our own operating manual - identifying what exactly supports and hinders our optimum functioning on all levels.


The work of the integrative physician is like that of a detective gathering as many clues as possible and then putting a jigsaw together.



Ecological vs Homeopathic Consultation

The process of taking/receiving an ecological history is somewhat different for that of a homeopathic history.  If you are clear which approach you are more interested in, please let me know at the outset as it will influence my approach and the type of information and understanding I am seeking. 



Please be clear that it is very rarely possible to obtain sufficient information for both approaches in one consultation, particularly when you first consult me as a new patient.



An Ecological Consultation

I will always wish to cover the all of following information:

  1. All your Symptoms and Problems in detail - including: diagnosis (if known), when each started, the circumstances and/or triggers when they started, to what extent they vary and what factors appear to influence them (make them better or worse) and whether they vary in unison with other symptoms.

  2. Previous Investigations, Specialist Assessments and Treatments - please bring copies (that I can keep) of investigation results and specialist letters.

  3. All your Current Medications, in detail - drugs, supplements, herbals, homeopathic medicines, creams, etc. and any other current therapies. Please supply: the actual name of the product, the manufacturer, the strength, the dosage you take, and the date started.

  4. Dietary History - your current and previous eating habits and the details and effects of any previous exclusion diets.

  5. Allergies, Sensitivities, Intolerances - drugs, vaccines, foods, inhalants (airborne), contact (skin), electrical (EMF).

  6. Environmental History - exposures to potential toxins.

  7. Microbiological History - previous infections, fevers, insect bites, etc - particularly recurring or severe ones.

  8. Psychological History and Sleep Pattern - all major or significant stresses and psychological traumas throughout the whole of your life.

  9. Physical Exercise / Activities - sports, running, walking, cycling, yoga, pilates, Tai chi and other martial arts, etc.

  10. Your Past Medical History - in a detailed succinct chronological form,  from conception to the present including all of the above.

  11. Your Family History in detail - this often provides some insight into possible inherited susceptibilities.

  12. A Physical Examination - the extent depends on your symptoms, but will usually include height, weight, pulse, blood pressure, eyes, nose, mouth, tongue, teeth, fingernails, range of spinal movements.


The paramount area to first discuss is usually your symptoms and problems and what you wish to achieve in relation to your health.

The other areas can be discussed in almost any order, but will often be determined by the nature of your problems.


You can now see why this process may take longer than one or two hours - and may take several consultations to cover all the ground!
